You'll also have a reusable template approach for your 30/60/90-day plan reviews.

Before typing, gather these details about your client (deidentified):

Type or paste this into ChatGPT, filling in the brackets:
Write a clinical treatment plan for a [gender/age range] in [ASAM level] for [substance] use disorder. Co-occurring issues: [list]. Stage of change: [precontemplation/contemplation/preparation/action]. Key strengths: [list].
Include 3 individualized treatment goals. For each goal:
- A SMART long-term goal statement
- 2 measurable short-term objectives with timeframes
- 2 counselor interventions
- Success indicators
Write in proper clinical language. Do not use the client's name.
What you should see: 3 formatted treatment goals with objectives, interventions, and success indicators.

30/60/90-day plan review update:
